Community-Based Services

We support people in a variety of settings such as their own home, at their parent’s home, at school, or in the workplace. The support provided is based on each person’s individual dreams and goals to enhance their lives. 

We are here to help people develop skills to be as independent as they would like to be. These services are provided to individuals diagnosed with either intellectual and developmental disabilities (I/DD) or mental illness.

Important note: Service availability is dependent on managed care organization (MCO) funding availability, diagnostic criteria and other factors that will be determined through Monarch’s screening process.
